Ebola, Outrunning the Response: A Warning From Congo That We Already Know How to Hear

I am going to say the unfashionable thing first. Ebola is still, in the epidemiology, a regional nightmare rather than a planetary one. That is not comfort. That is classification. Classification is how you keep a health system from lying to itself.

The World Health Organization has described the outbreak in the Democratic Republic of the Congo as spreading faster than the response can contain it. Al Jazeera reports 70,000 doses of Ervebo on the way. Those two sentences can both be true. A vaccine in a warehouse is not a vaccine in an arm. A WHO warning is not a movie poster. It is a professional way of saying: the maths have turned against us.

What “out of control” means in a clinic, not a headline

It means case finding is behind the virus. It means contacts are lost in displacement, funeral practice, or simple exhaustion. It means isolation beds, not press conferences, are the scarce resource. Ebola kills by fluid and by fear. Fear empties hospitals of other patients. That second death toll never trends.

Ring vaccination is a beautiful idea. It assumes you can still see the ring.

The list that actually matters

• Confirm the strain and keep confirming it. Ervebo is a Zaire-strain tool. Genomics is not academic here. It is the difference between the right vial and a placebo with a logo.
• Protect the protectors. Health workers are both the response and a transmission risk. PPE, pay, and a ride home are medical interventions.
• Funerals. If you cannot make safe burial feel like respect, families will choose respect. I would too.
• Rumors. In a week when rich countries are having televised arguments about routine vaccines, do not be shocked when a rumor in North Kivu finds oxygen.

I practiced long enough to distrust two kinds of outbreak coverage: the kind that treats Africa as a blur, and the kind that treats a vaccine shipment as a victory lap. Seventy thousand doses is a serious number. It is also a logistics novel — cold chain, security, community permission, the last mile of the last mile. If the ring cannot be drawn, the doses are inventory.

The rest of us can do something unheroic and useful: fund the boring parts, refuse racist metaphors, and keep our own vaccine politics from exporting contempt. Viruses do not need our contempt. They need a gap. We have given them those before. We know how this story goes when we are late, and we know how it goes when we are not. Choose the second one while there is still a choice.
